Another nail in the coffin of carotid endarterectomy.

چکیده
The randomized carotid stent versus carotid surgery trial reported by Brooks et al. (1) in this issue of the Journal is a notable accomplishment in several respects. By reporting results in 104 patients, it becomes the largest randomized carotid artery stent versus carotid endarterectomy trial published to date (to my knowledge). It is also an excellent example of team building with a surgeon, a cardiologist and a neurologist working together to optimize patient care. The authors are to be congratulated on conducting a randomized trial with prospective independent neurology oversight. The trial, as do all clinical trials, has strengths and weaknesses. The strengths include reasonably good methodology with routine preprocedural and postprocedural objective patient assessment by a neurologist and carotid duplex ultrasound scans within 24 h of the procedure and serially thereafter. Magnetic resonance imaging was performed at 6 and 12 months after the procedure to detect asymptomatic cerebrovascular ischemic events.

ANGIOPLASTY AND STENTING OF CAROTID ARTERY STENOSIS WITH EMBOLIC PROTECTION DEVICES
Background: Carotid artery stenting (CAS) has recently been recommended as an alternative to carotid endarterectomy (CEA) by some clinicians. Objective: This study was designed to evaluate the success rate and in-hospital and 30-day adverse events in our first experiences in Iran for CAS with protection devices, to document our results and guide further use of CAS. Methods: From December 2...

Background and purpose: Recommended treatment for patients with carotid stenosis, is Carotid Endarterectomy (CEA) that is more effective in preventing stroke compared to other therapies. The purpose of this study was to evaluate morbidity and mortality rate in patients with carotid stenosis after CEA. Material and Methods: In this descriptive study, 138 patients undergoing CEA admitted in the ...
متن کاملCarotid angioplasty and stenting in a patient with high grade stenosis of Internal Carotid Artery associated with both vertebral arteries and contralateral carotid occlusion
Severe internal carotid artery (ICA) stenosis is a common cause of cerebrovascular accident (CVA) in middle-aged patients. Contralateral carotid occlusion (CCO) in patients with severe ICA stenosis is associated with high risk of CVA. Carotid endarterectomy (CEA) is associated with more complications in patients with CCO than those without CCO. In this study, we present the case of a 61-year-ol...

PURPOSE To determine whether carotid stenting with embolic protection is equivalent to carotid endarterectomy (CEA) in a broad risk population of patients with symptomatic and asymptomatic carotid stenosis. METHODS A prospective, multicenter Phase I trial was conducted comparing standard CEA to carotid stenting systems (CSS) in patients with symptomatic (> or =50%) and asymptomatic (> or =75%...
متن کاملCurrent concepts in the management of carotid stenosis
Several large randomized clinical trials in North America and Europe concluded over a decade ago that carotid endarterectomy plus medical management was significantly better than medical management alone for stroke prevention in either symptomatic or asymptomatic patients with severe carotid stenosis. Percutaneous carotid angioplasty now represents yet another treatment option that currently ap...
